There is a 8-SOIC (0.154, 3.90mm Width) case package that contains the instrumentation amplifiers. There are two types of operational amplifiers in this circuit. Upon delivery, op amp ic is packaged in a Tape & Reel (TR) case. Surface Mount is the recommended mounting type for this linear amplifier.
